The Social Worker/Therapist is responsible for providing therapeutic support for transitional life events to patients and their families. Participate in the multidisciplinary team. Develops, implements and reviews treatment plans and reviews. Completes psychosocial assessments. Facilitates inpatient and post acute consults to meet psychosocial needs. Facilitates individual, family, and group therapy. Provides case management and discharge planning while documenting within required time frames. Ability to effectively assess, plan, and implement therapeutic services in a multidisciplinary team settings. Knowledgeable regarding all populations with background in medical social work.
Masters Degree in Social Work.
At least one year of experience as LMSW.
Experience in Medical Social Work in a variety of settings, utilization review, discharge planning, and resource management.
Demonstrated ability to work effectively with patients, families, physicians, and administrations.
To work well under pressure; to deal with multiple and sometimes conflicting priorities; and to utilize critical thinking skills.
The ability to process and willingness to learn new computer skills.
Demonstrated skills with computers and word processing.
